@DrEricDing Fundamental misconception below. That the virus has evolved as this is respiratory. Many things determine affected organs are affected in an infection eg mode of entry. No-one is saying that avian virus has evolved to infect udders. Just so happens there was an opportunity.
@PeterHorby Even W Euro region has seen a sharp rise in autochthonous cases in 2022 and 2023 and evidence of unreported cases in SE Europe suggesting shifting epidemiology. WHO announced a Grade 3 Global Emergency in December, though largely driven by increases in other regions.

My belief is that this bias is one reason for lower COVID mortality in some LMIC populations. Not only are there fewer elderly (DRC: 4% of 65+ vs Belgium 2O%) but also they are a lot healthier as those with underlying health issues did not make it to older age.
