Using a humanized mouse model, Victor D. Cuenca Narvaez @VictorCuenca3, Michele Barbarini @MBarbarini, and Sandra M. Herrmann @HerrmannMd & team @MayoClinic now show that TNF‑α blockade reduces kidney inflammation caused by immune checkpoint inhibitors, suggesting a potential strategy to prevent immune‑related kidney injury.

🔹 Prevalence: affects 25–60% of SLE pts; often within 5 yrs of dx, but can occur >10 yrs later
🔹 Children: 36–55% of childhood SLE develop LN; 5–20% → kidney failure
🔹 Ethnicity gap: Black (34–51%), Hispanic (31–43%), Asian (33–55%) vs White (13–23%)
🔹 APOL1 risk alleles → 26% Black vs <1% White; 2× ↑ risk & faster kidney failure
🔹 Mortality: LN → SMR ~7.9; leading causes = kidney failure, infections, CVD
🔹 Each LN flare = decades lost in kidney lifespan ⏳
🔹 Interferon signature (IFN-I): central in pathogenesis → predicts early, severe, resistant LN
🔹 Biomarkers: proteinuria, anti-dsDNA ↑, low C3/C4, anti-Sm, anti-C1q; emerging urine biomarkers 🔬
🔹 Pathways: B & T cell dysfunction, calcineurin activation, NF-κB, JAK-STAT, mTOR
🔹 Therapies:
•Dual therapy = GCs + MMF/MPA or low-dose CYC
•Multi-agent (better) = + BEL (belimumab), VOC (voclosporin), or OBI (obinutuzumab) 🚀
•Obinutuzumab trial → 46% renal response vs 33% placebo at 76 wks
🔹 Non-immune kidney hits: obesity, diabetes, salt/protein-rich diet, nephrotoxins worsen nephron loss
🔹 Repeat biopsies help distinguish active vs chronic damage → guide tapering & prevent under/over-treatment
🔹 Supportive therapy: RAAS blockers + SGLT2i → protect nephrons, slow CKD
🔹 Comorbidities: CVD, osteoporosis, fertility issues → must be proactively managed
🔹 Future direction: targeting tissue-resident memory lymphocytes & plasma cells = 🔑 to durable remission

📌 Bottom line: LN remains a top killer in SLE → early dx + strict adherence + multi-target therapy + holistic CKD care = best shot at kidney survival.
