ACP treating DKA. pH of 6.8. Absolutely no idea how dry the pt is. Her tongue, looked like a dry rock, 41 breaths/min to compensate for acedemia but ACP insisting on following DKA protocol and not using wit to give "extra fluid". ED consultant "supervising" him

@DrJoFranklin @DrNoOneSpecial I am sure HFpEF or HFrEF terms are aliens to many...I asked simple questions, not asking them have you done pocus? What was the ventricular contractilty like ? IVC size, Collapsibility ? Any other thing that would suggest hypovolemia ? But I guess, I knew the answers already.

@DrNoOneSpecial I asked how much fluid the pt has received ? Answer : 1L in 1 hour... for a patient with a lactate of 10 and BE -28 🤬🤬🤬
Another gem from ACP : pt also has AKI which is probably stopping her to respond to fluids🤦🤦🤦.
Not even contemplating what caused this AKI ....
