Cheers, chills, and a standing ovation when RASolute 302 showed unprecedented survival on daraxonrasib for patients with progressive pancreatic cancer
Seldom do you sense you’re witnessing a historic moment in cancer care but this feels like ras targeting has arrived
#ASCO26
Presented at #ASCO26:
Among patients with previously treated metastatic pancreatic ductal adenocarcinoma, the RAS(ON) inhibitor daraxonrasib led to significantly longer overall survival and progression-free survival than chemotherapy. Full phase 3 RASolute 302 trial results: https://t.co/xwLWBZYRzq

📜 Anticoagulation for PVT in cirrhosis
🗣️ N. Riva |
💡 Treat based on: LT candidacy, thrombus extension & cirrhosis severity
🔬 DOACs in cirrhosis:
✅ Child A → all DOACs OK
⚠️ Child B → apixaban/edoxaban with caution, avoid rivaroxaban
❌ Child C → avoid all DOACs

A GI numbers pearl:
👉 1 mm = 3 Fr
So:
7 Fr = 2.3 mm
10 Fr = 3.3 mm
A 7 Fr device fits every adult GI scope.
A 10 Fr device needs a ≥3.3 mm working channel (therapeutic gastroscope, adult colonoscope or duodenoscope).
Save yourself a trip to the processor cart.

Behold the “Megacap” ie @OvescoEndoscopy RemOVE FBR Kit.
Remove food bolus en bloc by suctioning rather than pushing blindly inward. The large soft cap allows large volumetric capture and folds to allow for passage down the oropharynx easily.
The long cap prevents the bolus from jamming the suction channel (as it would short caps). When you see “food out, during suctioning, start to wiggle out and reveal your prize!
❄️Cold resection techniques match hot EMR for 10–20mm #SSLs
🔸Cold snare polypectomy(CSP) & cold EMR achieved non-inferior resection vs hot EMR
🔸CSP reduced procedure time
🔸Submucosal injection improved en bloc resection & margin

Robotic endoscopy in action! Mounted over the scope, the robot has an arm that is controlled externally, with the ability to rotate, flex and grasp tissue. This enables countertraction to assist with complex resection of neoplastic GI lesions.

Identification of liver segments on EUS relies on vascular anatomy: Portal vein & Hepatic vein.
To recognize them:
-Perpendicular to one other ie. if PV is appearing longitudinal, HV will be circular
-Flow on doppler
-Crucial for identification of liver segments

Bacteria in our BRAINS??!
We observed translocation of bacteria from the gut to the brain via the vagus nerve.
This occurred after high fat diet, or without dietary changes in mouse models of Alzheimer’s, Parkinson’s, and autism.
