An observational cohort study on the effects of extended postoperative antibiotic prophylaxis on surgical-site infections in low- and middle-income countries

This cohort study evaluated whether postoperative antibiotic prophylaxis decreased the risk of surgical-site infections in 19 hospitals in low- and middle-income countries, where postoperative prophylaxis is common for wound class I and II operations.
🚫Giving postoperative prophylaxis for greater than or equal to 24 h was not associated with a lower risk of surgical-site infection 🚫
It was associated with a longer length of hospital stay, in resource-limited healthcare systems. With the growing threat of antimicrobial resistance, surgical initiatives to implement antimicrobial stewardship programmes in low- and middle-income countries are critical.

Abdominal drainage after elective colorectal surgery: propensity score-matched retrospective analysis of an Italian cohort

The current study examined the impact of abdominal drain usage after elective colorectal surgery in Italy.

Analyzing 6157 propensity score matched patients, a significantly lower risk of postoperative duration of stay >6 days with a mean difference of 0.86 days was noted for patients w/o drains.
No difference was recorded for all the other endpoints, including all analysed complications.

☝️Abdominal drains should not be used routinely in colorectal surgery.

How do we work up an adrenal incidentaloma?

It depends on the Hounsfeld Unit (HU) density on a simple non-contrast CT scan:

If HU < 10, you have 100% ruled out a malignancy!
