A 40 year old male who has a bone marrow for investigation of splenomegaly with mild anaemia and thrombocytopenia. Hb 125 g/L and Plts 112x10^9/L. Normal MCV 91 fL and MCH 29.3. WCC 7.1x10^9/L and diff normal.

Pt with a hx of CLL now presents with increased lymphadenopathy. Occasional small lymphocytes are seen but larger nucleolated lymphocytes are present. Film show lots of abnormal forms often with nuclear budding. #richterstransformation #hemepath #haempath #CLL

Another uncommon finding in a patient with new provisional diagnosis of AML with Maturation. See the blast and ring neutrophil. I rarely ever see ring or doughnut neutrophils. #hemepath #haempath #AML #bloodfilm #hematology #haematology
