You have a kid in septic shock... What MAP target do we aim for?

A study published this month randomized kids in septic shock to the 5th percentile mean blood pressure for age versus the 50th percentile. There were 144 kids randomized in an open-label design using art lines or non-invasive BP measurements. The gist of what they found:
-No difference in survival
-No difference in CKRT use
-More ARDS in the higher target group
-Trend towards more digit ischemic and feeding intolerance in the higher group

Take home point: Given the risks associated with vasopressors, we should probably aim for lower MAP targets.

On prehospital cricothyrotomy in the US... this study published in June 2025 found:

-Incidence of 1.9 per 100,000 EMS calls
-Nearly half were performed in the setting of cardiac arrest
-Most were performed on males <60 years
-Successful placement was 83%

Of note, they excluded air EMS services and interfacility transfers.

While this study is interesting due to the sheer massive size of the dataset, it still leaves a lot to be desired. We do not know what other airway interventions were attempted and failed, and importantly, why those airways failed. We also have no data on the number of attempts or what the "failures" were in those 17% including complications. That said, a neck laceration or other complications are far less of a problem than an anoxic brain injury.
