@MichaelMegalyMD In my experience, penumbra usually works well in fresh clot situations but doesnt work as well in more ogranized thrombus. Also cost is its biggest issue imo. Otherwise, I love using it in the right case!

Recent case of a middle aged pt with HCM
Echo performed late morning demonstrated no LVOT obstruction (fasting).
Patient returned for consultation following lunch & was noted to have loud, harsh 4/6 SEM.
Doppler study was repeated which demonstrated severe LVOT obstruction!

@NosairMohamed @LAzzaliniMD How did the left coronary system look? Would consider CABG is significant LAD/LCx dz. Otherwise, laser with contrast or brachytherapy would be options to consider. Not sure how effective IVL would be in this scenario.
