I have always initiated medical insurance discussions with people close to me at the earliest possible opportunity. Some get the point and take action, while others ignore probably thinking I am simply looking for business, only to reach out a few months later when they now need the cover 'urgently.' At this time, they mostly have sensed an unavailable risk. Maybe one is already unwell and at the hospital and they project a huge bill, or a pregnancy has occurred and now one needs cover for maternity, or there is a scheduled procedure anticipated to cost huge sums of money.
Unfortunately, medical insurance has waiting periods for certain conditions including normal in and outpatient visits, pre-existing conditions, maternity, psychiatric conditions, newly diagnosed chronic conditions among others. As a result, it becomes practically impossible to take up a cover to fix an 'urgent' medical bill. While this is not good news, it is the price for not taking action when the time was right. Insurance companies, like all other business entities hope that no claims come, and therefore has strategies to avoid outright risks.
It is therefore advisable to take up cover before any red flags regarding your health. The earlier you sign up, the cheaper it is for you, and the more you are assured to get protection when you need it most.

And in case you have financial limitations and is unable to afford both in and outpatient cover, it is possible to have an inpatient only cover which is way cheaper. The biggest hospital bills arise from admissions, therefore a basic inpatient cover will save you a lot of money.
