Drug companies have spent millions pushing the narrative that middlemen like PBMs are the primary driver of high drug prices. That messaging push has gained traction, and PBMs are responding with increased advocacy of their own. While PBMs’ opaque and profit-driven practices play a significant role in patients’ access and costs, drug prices are ultimately set by manufacturers. The result is a system where powerful industry players continue to shift blame, while patients face the consequences of high and rising prices. Meaningful reform will require addressing the full system — starting with the pricing power of drug companies — to deliver lower costs for patients.
Pharmaceutical corporations remain the biggest driver of high drug prices. Drug companies set prices at launch, raise them over time, and abuse the patent system to block lower-cost competition. To meaningfully lower costs for patients and taxpayers, we must address these practices while eliminating the perverse incentives that allow PBMs to profit from higher-priced drugs. As debate continues around new proposals to lower prescription drug prices for Americans, one thing is clear: the 2022 prescription drug law is already helping to lower costs for millions of patients across the country.
As of January 1, 2026, all major provisions of the law are in effect. What began as a series of phased-in reforms has lowered out-of-pocket costs, improved access, and brought greater predictability to millions of Americans.
