@sscpCCEmpsn Balanced crystalloids (e.g., Lactated Ringerโs or Plasma-Lyte). SSC recommends crystalloids as first-line fluids and suggests balanced crystalloids over normal saline for initial resuscitation in septic shock.
But patients arenโt equations and medicine isnโt always black and white.Sometimes the hardest part isnโt finding the answer, itโs deciding which answer to trust.
Thatโs what makes medicine challenging and humbling ๐ฏ
I sometimes wish clinical practice were as simple as math.
In math, 1 + 1 always equals 2
In clinical practice, the same evidence can lead to different decisions, and the right answer often depends on the patient sitting in front of you
REMAP-CAP Oseltamavir arm:
Let me preface this by saying that I've never been a big oseltamivir fan (seriously, who is?).
I'd love to see oseltamivir proven ineffective so I'd never need to argue about it ever again.
But I don't think the math here works.
Applying a Fisher's exact score to the mortality difference yields a p-value of 0.17 which is nowhere *close* to statistically significant (by any standard).
I'm increasingly tired of seeing these studies where the p-value is ginormous but the study claims to be a slam-dunk (98% risk of harm!).
Stopping a trial prematurely for harm with a p-value of 0.17 will ultimately lead to a lot of confusion about what this study means (spoiler alert: probably not much).
Hopefully the manuscript will clarify this, but the math doesn't seem to be mathing. If you're going to claim 98% certainty of a result, then the results should be consistent regardless of how you run the statistics.
Just to be clear, I despise oseltamivir, but I'm worried that we're going down a slippery slope when a Bayesian wonderland somehow converts p=0.17 into 98% certainty.

Iโve lived long enough to know that not everything needs to make sense right away.
Some things reveal their meaning only after the pieces come together.
