intentionally mapping focal AT inside LEFT MAIN with a Pentaray spline 😱... redo case... similar ablation as first one was performed, but much longer time, to reach epicardial origin as suggested by good left main atrial timing. #Epeeps
See that middle bipolar Pentaray signals (wider interelectrode distance) in LA suggest prepotential as a far-field signal
ATYPICAL ATRIAL FLUTTER post PV ablation‼️. The danger of leaving 2 gaps in a WACA 😱. Carto v8 displays activation, automatic tracking of fragmented EGMs and local slow conduction, paving the way for a simple focal ablation...more mapping and understanding, less ablation... #Epeeps
So we looked at velocity vectors, with emphasis in the slow ones (we applied a <0.2 m/s filter): although volatje seemed normal, a region of slow conduction could be seen in the posteroseptal region 🧐
Radiofrequency applications there induced slow nodal rhythm. No more slow pathway could be seen afterwards and no other arrhtyhmias were induced 🌈 Thanks to @Raulramesc for his support in the case!
