73 year old man with high burden of monomorphic ventricular ectopy (32%/day). LVEF 52% (previously 65%). Earliest activation in LV ostium (-27 ms), in close anatomical relationship with RCC-LCC interleaflet triangle.

Tachycardia was finished with applications in the isthmus from pericardial patch to pulmonary valve stent (mesh). Aditionally, we performed lesions to complete block between tricuspid annulus and RVOT (functional block during VT, not present during LV pacing map)
Incessant slow #VT with intramural circuit in end-stage HCM despite previous #VTablations. Coil insertion in a small branch of 2nd diagonal coronary artery. CT integration in the #EPlab❤️ at Miguel Servet University Hospital. #EPeeps@EPeeps_Bot@secardiologia@escardio
