How to prepare for USMLE Step 1 ethics questions
Start by learning the basic rules. First Aid, Boards & Beyond videos, and others cover these well. Know what autonomy means. Know the components of informed consent. Know not to treat friends or family unless it’s an emergency. Every student learns these before the exam.
But beyond that, you need to master these three hidden rules:
1. Never shame the patient.
2. Never threaten the patient.
3. Always build a bond with the patient.
They seem obvious but they’re often buried in tricky answer choices.
Example - suppose this is an answer option: “Ask the patient why they’re so worried about pain when the test is normal.” Looks reasonable. But it subtly shames the patient for being anxious. That’s a trap.
Or: “You need to quit smoking or you’ll die from COPD.” That’s a threat even if it’s medically accurate.
Now compare: “I know it’s hard to quit drinking. Can you tell me more about why you’d like to stop?” This builds a bond. It shows empathy. Answers like this are often correct.
Final tip: Ethics questions are hard to write. Lots of bad ones are floating around in Qbanks (I’ve written some myself). If you strongly disagree with an explanation, don’t obsess. Learn what you can and move on. If you are getting *most* of your ethics practice questions right, you're doing well.
Bottom line: Learn the basics. Learn the 3 hidden rules. Then practice. Over and over.

In 2022, only 31% of the population in sub-Saharan Africa had access to safely managed drinking water services.
Achieving SDG targets in low-income countries by 2030 will require current rates of progress to increase by 20 times for basic water services.
Unmatched before 2024: "Your scores missed the mark"
Unmatched in 2024: "Other candidates did a better job 'selling' themselves"
Match 2024:
⇈ # of candidates went up
and... scores mattered less
This means you'll lose out on opportunities
↪ Not because of scores or you're lacking experience
↪↪ But because another candidate did a better job presenting theirs
Sometimes, just by a hair.
So, how do you bypass this?
In my opinion:
- Build connections
- Narrow down your search
- Focus on speciality specific programs
- Refine your interview skills by practicing early
- Take Step III if you haven't, ⇈ USCE, and publish
- Rebrand your CV and PS narratives (this is a big one!)
Instead of letting 'Imposter Syndrome' creep in
Adapt your approach and try again.
*It's hard to beat a candidate that doesn't give up.*
