Chinese Expert Consensus on Low-Dose Radiotherapy
🔹 Optimal dose: 0.5–2 Gy/fraction (≤10 Gy)
🔹 ICI timing: LDRT 1–7 days before or concurrent with ICIs.
🔹 Novel strategies: LDRT+SBRT for multiple lesions; SBRT embedded within LDRT for bulky tumors.

Six year's work!!! Finally published. Heterogeneous RT-(SBRT embedded in low-dose RT plus aPD-1 (immuno-EclipseRT, iERT)) elicits CD8t T cell immunity against bulky tumors via an IFN-I/NK/DC axis.

Immuno-EclipseRT (iERT), which combines low-dose RT to the whole tumor, partial-tumor SBRT and PD-1 blockade, controls bulky tumors via an IFN-I driven XCL1⁺ NK/cDC/CD8⁺ T cell axis. iERT is safe and effective in patient's (>100 pts). #ESTRO26

1/3 Our work just published in Signal Transduction Targeted Ther! Combining low-dose radiotherapy (LDRT) to bulky lesions, high-dose SBRT to small lesions, and PD-1 blockade, showed encouraging durable survival (mOS 51.3 months) in advanced NSCLC.
2/3 Mechanistically, LDRT induced IFN-γ and GM-CSF to program TNF-α⁺ antitumor neutrophils, while CXCL–CXCR2 signaling drove them to distant tumors. There, these neutrophils supported CD8⁺ T-cell function through an ICAM-1–LFA-1 axis within a specialized neutrophil–CD8 niche.

Thanks @AngelaHong!! We could not have completed accrual to our trial (in 6 years!!) without your amazing accrual in Sydney and by our other colleagues in Australia.
