Never forget, coronary arteries fill during diastole.

The gradient is defined by

1. Diastolic AoP at the “top of the hill” and

2. LV diastolic pressures at the “bottom of the hill”

A patient with low MAP and very high LVDP / PW may have coronary ischemia.

Unload the LV.
