Engage your friendly neighborhood vascular neurologist before LAAO if the indication is prior ICH or CAA. Better yet, send them to the @MayoClinic Heart-Brain Clinic. Risk stratification is essential.
Probable CAA is a very heterogenous population, and in select cases we don’t use anticoagulation in setting of high burden AF. Prior ICH and cortical superficial siderosis are significant risk factors for recurrent ICH, in which we may consider LAAO.
Not noninferior = INFERIOR
Take your blood thinners folks - still the gold standard for stroke prevention in patients with AF who don’t have clear contraindication to anticoagulation.
Among patients with atrial fibrillation at high risk for stroke and bleeding, left atrial appendage closure was not noninferior to medical therapy in reducing the risk of stroke, embolism, major bleeding, or death at 3 years. Full CLOSURE-AF trial results: https://t.co/qAMC2o36Mi

Patients with the largest infarct burden showed the most benefit with early re-start of AC. We should avoid hesitancy/bias from "big-bad, looking infarcts" and stick to the evidence: Starting earlier is better. Algorithms won’t replace stroke experts but they might be able to improve mimic detection by ED or EMS personnel. No one TS mimic score stands alone but provide complementary info.
