43 yo W acute onset of right CN III palsy.
First symptoms 7d PTA
Felt like something burst behind her right eye 2 days ago. Near complete right 3rd (not down and out, but complete ptosis and only lateral movement).
Negative SAH (on CT). HA not "WHOL".
So many questions?
@stephanamayer I think puncture to recanalization time is a surrogate measure of disease severity, procedure complexity, and futility. More passes and difficult to revascularize. An expected lead indicator for re-occlusion. Complete revascularization more important than time.

Love this. I hope the @BidenHQ campaign rips this gag video off for an ad. Maybe have @JimmyKimmelLive or @Rob_D_Niro or some other celebrity perform it. Please watch.
Even more evidence that door-to-needle time for BP control in acute #ICH matters.
In this pooled analysis of RCT data (>5000 pts) earlier and sustained SBP control <140 was associated w/ better outcome

No benefit of TNk compared to standard of care for mild (non disabling) stroke presentations with intracranial occlusion/CTP deficit. Definitive answer to this patient population.

60ish male with 3 day hx CRAO, Lt embolic strokes, and >50% stenosis (post bulbar, RadX for CA). Early CAS, discharged Home next Day, mild improvement in L Eye Vision (recognize fingers, no Acuity), otherwise Neurologically Intact. Early revascularization best. BEFAST

@TudorGJovin@AdamArthurMD@AdamArthur Also MR Diffusion is not binary and has a gray scale that may be reversible. Nevertheless, exciting work demonstrating an opportunity space for future improvements, perhaps with novel neuro protective or resuscitation strategies? I still believe patient by patient judgment needed
@TudorGJovin@AdamArthurMD@AdamArthur From the mRS shifts, looks like 6 to 3-5 (not 0-2, which are equal in all groups). I question the definition of infarcts in these trials with CT ASPECTS highly inter-observer variable, often mild edema gets the same point by off as profound hypodensity.
Multiple Aneurysms are often observed in SAH patients. Often, it is difficult to determine which aneurysm has ruptured and is most Urgent to Treat. In these Cases, treatment of Multiple Brain Aneurysms at 1 time is occasionally required, and should be strongly considered!
Symptomatic ISR of Carotid Stents remains uncommon: Drug Eluting Balloons have demonstrated improved Long-Term Patency in the Peripheral Vasculature as well as smaller Coronary Arteries, an exciting option for unusual high risk situations in the Cerebral Vasculature. (2 year FU)
