Accumulating 2 weeks a year is very common as a starting employee, though it has trended higher in recent years. My company gives all employees 4 weeks up front. With a separate 2 weeks of holiday and 1 week of sick.

The cost that "insurance pays" is a made-up number negotiated between the insurance company and the provider. It's meaningless and not a number anyone ever sees. Uninsured pay a different number.

If you haven't actually planned a payoff before simply throwing out a bunch of "setups" then the rest of your story won't reflect those payoffs.
