Osteotomy should be started at least 35 mm distal to the tibialplateau to prevent postoperative valgus knee laxity.

@1gregorross @bellkneesurgeon @kneeguyuk @DrJN_SportsMed @kneedoclond @clairepatella I have seen a few of these and if CT shows implant position to be reasonable a MPFL has been helpful and resolved symptoms in patients with delayed PFJ instability following PFJR. Revision to TKR is always an option, but physio alone probably won’t do the job.

This degree of anterior sagittal laxity is rarely consequent to a simple ACL disruption.
Associated medial and lateral meniscal lesions (fortunately both salvageable) in this case.
Complex meniscal preservation surgery takes longer than the ACL reconstruction..

[1/12] Complex closed plateau with comminuted metaphysis/shaft component. Brace yourselves— lots of steps to get this right if plating. Right off the bat you need to decide, will you bridge or direct fixation? Ring fixator? My path was direct plating BUT other options fine too.
