In January 2023, I was on a night shift & was woken up by the nurse at 5:00 am. We had received 3 patients.
A 10 yr old girl, brought dead
Her mother, in her 40s with long bone fracture
And her grandmother with maxillofacial injuries.
Obviously they had been in an accident.

Emerging evidence suggesting that keratin-based hair straightening products containing glycolic acid or glyoxylic acid is associated with development of kidney injury and chronic kidney disease.
Glycolic acid is a byproduct of the oxalate pathway and comes from the conversion of glyoxylic acid.
The injury is due to development of oxalates inside kidney that damages the organ after absorption of the compounds through scalp/tissue surfaces.
This requires further validation from larger series of patients.

I had a cup of chai and left home to pick the kiddo up from preschool. Found a cab whose cabbie was having chai. Passed by our neighbourhood grocery store where the owner and workers were having chai. Reached the school, the door opened and saw the principal was having chai. Reached my building and the watchmen were having chai. Reached home and mom said, "I am making chai, will you have some?"
The entire country runs on chai 🇮🇳

Dr. Pal gets it wrong again and in the process, disparages mothers who opt for C-section instead of normal vaginal delivery
I am a gut microbiome clinical researcher.
https://t.co/sDs0dwkLzG
And it is my duty to correct health misinformation especially if it falls in my domain
Claims:
1. Vaginal delivery improves gut bacteria in babies rather than C-section delivery
2. Babies born through normal vaginal delivery have better future health (as in less obese) than those born through C-section
Does that mean moms opting for C-section are to be demonized for their kids poor health in the future?
Absolutely not.
This is misinformation.
Response:
1. Two new findings:
(a) Older studies mention that babies born through vaginal delivery have different bacterial profile when born through C-section. That is the microbiomes of C-section babies look a lot different from those of babies born vaginally. But the impact of this on future health of babies remain unclear - meaning there is no signficant impact on babies health in this context. But new studies have shown that whether born naturally or via Cesarean section, babies receive essential microbes from their mothers. So dont worry about the type of delivery.
"While cesarean-born babies do receive less of their mother's gut microbiome during birth, they make up for this by drinking their mother's microbes in breastmilk."
https://t.co/4rxddTdV6l
The study found that about 80 per cent of C-section-born babies had hospital-acquired bacteria in their guts when they were born, compared with 50 per cent of vaginally born babies. But by the time the babies were weaned at the age of around 6 to 9 months, these differences had largely disappeared. And all the babies were healthy, so the researchers can’t tell if there are any implications for health.

(b) But hear this out - New research suggests that exposure to the vaginal microbiome during birth may not influence babies' gut microbiome as has long been assumed. A new study conducted by a team of Canadian scientists has revealed that the composition of the mother's vaginal bacteria has no significant influence on the bacteria composition found in infant stool during early life.
https://t.co/btbEAbZIFv
"It does not appear that exposure to maternal vaginal microbiota at the time of vaginal birth establishes the infant stool microbiome", - Dr. Deborah Money, Professor of Obstetrics, University of British Colombia

2. There is ABSOLUTELY NO link between C-section deliveries causing obesity later on in life. This has been debunked by well-designed studies recently. And there is no proof that the changes associated with gut bacteria due to C-section vs vaginal delivery promotes obesity in kids in the future. Absolutely none.
https://t.co/dmY0fByn2Q and https://t.co/S0DLMrCb9r and https://t.co/OC6Af5q0gU
A woman wants C-section vs normal vaginal delivery? It is her choice. And then there are medical reasons to opt for C-section too. C-sections is all fine. So my dear ladies, please dont get gas lighted into feeling sorry for yourself. You take care.

@scalpeltalks One of those days at work where you feel you aren't doing enough and self-doubts kick in😕.For me, it's unwavering dedication towards my passion, but the same commitment can become a cruel burden when one must make impossible choices or witness the limits of it.
They were once in love.
When he had a heart attack she was the one he called.
She brought him to hospital.
Saved his life.
Stepped out and was standing near the gate crying.
I stopped the car when I was leaving the hospital.
"He will be fine!" I told her.
Her eyes lit up. "Thank you" she murmered.
He's engaged to marry someone else.
She says she's his well wisher.
I say she's his guardian angel.
And he's a fool to leave her.
Life is such.
I look at her gift.
I wish and pray for her happiness! ❤️❤️❤️ #LoveHurts
European village, cozy cottage, English breakfast, and a La La Land date with Ryan Gosling.
But alas, duty calls! Back to work with a sigh, but hey, a girl can dream, right? 😉
@Badass_Superdad Imagine paying so much for a cab, racing to the airport like a maniac, only to reach the gate and realize your flight just flew away, waving at you like "See ya, sucker!"
I have received this "Ayurvedic Medicine" prepared and prescribed by an Ayurveda practitioner from Karnataka which was given to a woman for treatment of fatty liver disease.
She had Gilbert's Syndrome, a benign cause of mild jaundice which does not require treatment. After using this Ayurvedic formulation for a week, her jaundice has increased to 14 mg/dl from previous 3mg/dl with severe hepatitis. She underwent a biopsy and is now on close medical care, trying to prevent liver failure.
My lab is now tasked with the duty of identifying what components this special formulation has. All we know is that the vehicle-base is "jaggery."
Be aware of these conditions that do not require treatment, but Ayurveda and Homeopathy practitioners will jump at every oppurtunity to treat them, because they can make money by defrauding patients into believing they need treatment.
1. Gall bladder stones without symptoms.
2. Kidney stones without symptoms.
3. Gilbert's syndrome (benign cause of jaundice)
4. Any hernia without symptoms
5. Seasonal allergies that require only short course of evidence based medical management.
6. Childhood asthma or allergies that require symptom and preventive care as per evidence based guidelines - majority of children grow out of allergies and asthma after a particular age.
7. Ultrasound diagnosed fatty liver - grade 1,2 or 3 does not have clinical relevance, they are all the same, in the absence of scarring and inflammation in the liver (identified through further testing - which alternative med practitioners have no clue about)
8. Seasonal viral infections (flu) - can be prevented with vaccination and are self limiting, symptom care is only what is needed, not long term unnecessary herbal or Homeopathy treatments.
Unlabelled formulations, multiherbal formulations, multiple herbal treatments, mislabelled herbal formulations, herbal and dietary supplements - beware of these, so that you can safegaurd your own health. Take care.
MBBS is a tough course. There is a huge amount of knowledge one has to gather, assimilate and apply. Although I had been a good performer academically and got into it through merit, MBBS opened my eyes and humbled me right in the first year. It can be disconcerting to be at sea and lost!
