Reactive intralymphovascular immunoblastic proliferations➡️ Recurrent theme in #Hemepath consultations sent from other #surgpath services
Cases with these proliferations are typically sent by #GIpath (most commonly in resected appendices or GI segments for obstruction) or #ENTpath (tonsillar tissue as seen in example below)
Histologic sections show focally expanded lymphovascular spaces filled by B-immunoblasts that may mimic aggressive lymphomas
These B-immunoblasts demonstrate a postgerminal center immunophenotype, with positivity for CD38, CD43, CD45, CD79a, and MUM1/IRF4, and a high Ki67 proliferation index

The promyelocyte is the biggest cell in the neutrophil series. It also has huge, dark purple, primary (azurophilic) granules both in the cytoplasm and overlying the nucleus. However, it does NOT have the beginnings of secondary (specific, pink, salmon-colored) granulation!
