"And Yes", Pancreatic Neuroendocrine Tumors (PanNETs) can have rare intranuclear inclusions/INCIs (at 3, 6 and 9 o'clock). With these INCIs, the differential would also include a Solid Pseudopapillary Neoplasm. I have personally seen rare INCIs in p-diff Acinar Cell CAs as well.

Most common primary hepatic sarcoma (1-2% primary liver malignancies)

A urine specimen with a golden brown perinuclear pigment, consistent with lipofuscin.The differential diagnosis includes seminal vesicle cells (as in this case) and bladder lipofucinosis. The cells have enlarged focally irregular nuclei. A pitfall for the diagnosis of malignancy.

"From this morning's sign-out" - Abdomino-pelvic washing. We all often see the so-called "String of pearls" appearance of benign mesothelial cells. Here is an almost real "Serpentine".
"A Soapy Smear" - Despite the deceptively innocuous look of this adrenal FNA, follow-up histopath revealed an adrenal cortical carcinoma. I almost always report these cases as "Adrenal Cortical Lesion" and leave it to the tissue dx for further characterization.
"An Imposturous Thyroid FNA" - Despite the "WHOPPING" INCIs (inclusions on steroids) and oval nuclei really simulating a Papillary CA, this aspirate is actually from a Medullary CA (MTC). Mega inclusions can be a feature of MTC.
