William Osler (1905) remind us not to listen too closely to the chronic complainers in medicine, as “there are such in every calling” and the source of discontent usually comes from within.
Julie, a good learning experience. These 2,050 calories will cause a massive insulin spike followed by a post-meal crash. Increased hunger 3-4 hr later. Your arteries constrict and blood vesselsl stiffen. The nitrites are a group 1 carcinogen (increased colorectal cancer risk). Acute oxidative stress. Impaired endothelial function by 20-40% for several hours. Gut microbiome will shift towards pro-inflammatory Firmicutes, an obesity-like profile. The dopamine spike from fat+sugar+salt mimics an addiction reward pathway. Repeated, will desensitize dopamine receptors causing you to escalate stimulation to achieve the same pleasure.
Happy Birthday.
@rbarbosa91 Medical training is an acceptable length, the requirement of a conventional 4 year degree is what should be reformed. This would address debt burden and the physician shortage.
A surgeon in China successfully removed a lung tumor from a patient located 5,000 km away by operating a robot remotely from Shanghai. The innovative procedure took place with the patient in the city of Kashgar, and the operation was a success.
When I was in elementary school, my mom started going blind. Retinitis pigmentosa. No family history. No treatments. No cure.
I got lucky. She didn’t. It led me to build @OrchidInc so my baby —and everyone else's—gets to win the genetic lottery—avoid blindness— and hundreds of severe genetic diseases.
Today, the New York Times covered the tech we’ve spent years building:
Whole genome embryo screening for *hundreds* of diseases.
Not in theory. Not in mice. In humans. In IVF centers. Right now.
One of my biggest regrets was voting for President Obama in 2008 as a medical student & celebrating the passage of the ACA
Little did I know the carnage it would wreck on the medical profession and my future patients. I had no idea then that the ACA was written to strip physicians of their autonomy, boost insurer profits & drive healthcare consolidation
End result 15 years later is a more expensive healthcare system with less competition resulting in lower quality, less efficient care for patients. Coverage is not care!
Today marks the beginning of OpenRxiv, which replaces bioRxiv and medRxiv, the world's largest preprint platform for life and medical science @openrxiv
Agency > Intelligence
I had this intuitively wrong for decades, I think due to a pervasive cultural veneration of intelligence, various entertainment/media, obsession with IQ etc. Agency is significantly more powerful and significantly more scarce. Are you hiring for agency? Are we educating for agency? Are you acting as if you had 10X agency?
Grok explanation is ~close:
“Agency, as a personality trait, refers to an individual's capacity to take initiative, make decisions, and exert control over their actions and environment. It’s about being proactive rather than reactive—someone with high agency doesn’t just let life happen to them; they shape it. Think of it as a blend of self-efficacy, determination, and a sense of ownership over one’s path.
People with strong agency tend to set goals and pursue them with confidence, even in the face of obstacles. They’re the type to say, “I’ll figure it out,” and then actually do it. On the flip side, someone low in agency might feel more like a passenger in their own life, waiting for external forces—like luck, other people, or circumstances—to dictate what happens next.
It’s not quite the same as assertiveness or ambition, though it can overlap. Agency is quieter, more internal—it’s the belief that you *can* act, paired with the will to follow through. Psychologists often tie it to concepts like locus of control: high-agency folks lean toward an internal locus, feeling they steer their fate, while low-agency folks might lean external, seeing life as something that happens *to* them.”
Boss: You get that video ready to show off the world’s first bipedal musculoskeletal android?
Video guy: Sure did boss, strung its hideous bleached body up on wires like one of those angel kaiju from Evangelion. made it a Francis Bacon painting, just like you asked
Boss: what
@sailaunderscore egg whites + 2 whole eggs and double scoop protein to go gets u close to 90 grams in under 20 minutes of prep/ cook time. Greek yogurt , chicken and rice gets you rest of the way there. Its not exactly exciting but it gets the job done with minimal cooking and 3 meals.
