@roropsych The top relative to your hand position when held as I described, as is intuitive for most people, not the top in some absolute or metaphysical sense.

It definitely isn't. I worked for a long time at a power plant built in 1970 and all of the boiler ducts used turning vanes, large radius bends, etc. For commercial and residential, these improvements aren't visible and would increase building costs. There's an enormous amount of efficiency lost because the end user isn't thinking about this stuff, wants a cheaper structure, and will ultimately incur the ongoing costs of running a worse system.
A not-insubstantial portion of our decision to stop at one was my wife's intense and specific memory of the pain. She's an outlier along a few dimensions though. Home birth, no meds, crowned for 90 minutes with a particularly narrow frame and 10+ lb baby. Between the intensity and exhaustion, she was losing consciousness between contractions for the last hour or so. The midwife remained calm, but definitely switched into Serious Business mode and told us afterward that she had been close to calling an ambulance.
