Gabapentin (FDA approved for seizures & neuralgia) may be useful in AUD & alcohol withdrawal d/t GABA-glutamate balance restoration. 2018 APA guidelines endorse gabapentin as 2nd line tx for mod-severe AUD as it had small (+)effect on EtOH outcomes w/ little tx harm. 1/2

#OTILT Sublocade (buprenorphine ER SQ inj) is used to tx mod-severe OUD. Manufacturer recommends pts receive transmucosal BUP 8-24 for ≥7 days as pre tx. After SQ inj, med forms depot which releases BUP at a controlled rate x1 month. 1/2

#OTILT 1/3 Opioids can be safely used to treat pain, however, escalating/binge pattern use over a longer period of time can produce feelings of pleasure/euphoria by triggering the VTA to release excessive DA into the NA 🧠 (positive reinforcement)
