How do you keep control during a coronary perforation?
Dr. Sripal Bangalore (@SripalBangalore) and Dr. Hady Lichaa (@HadyLichaaMD) share their approach to rapid stabilization, the role of bedside echo and pericardiocentesis, and why covered stents must be sized and deployed with precision to seal the leak for good.

Going past forty atmospheres in a coronary was once considered reckless.
Dr. Deepak Kapoor tells Dr. Hady Lichaa (@HadyLichaaMD) why a balloon that does not grow changes that calculation, and the three places he reaches for one: vessel optimization before stenting, in-stent disease where expansion is the goal, and lesion preparation before drug delivery.

Technical tip on sequential stent deployment: given more complex left iliac angulation, some cases benefit from deploying the right stent first.

67 yo male with severe bilateral iliac vein stenosis and no infra-inguinal DVT on ultrasound. What do you rely on most to determine stent sizing and landing zones? (imaging in thread)
Reply with how you’d choose your landing zone, especially if you’d do it differently.
Specialty balloons create controlled dissection, not just more force. Dr. Ziad Ali (@ziadalinyc) and Dr. Nyal Borges (@nyalborgesmd) cover device selection, undersizing in calcium, and why the first device in a lesion matters most.

Would you flush a guide extension during PCI without thinking twice?
Dr. Sripal Bangalore (@SripalBangalore) joins Dr. Hady Lichaa (@HadyLichaaMD) to explain why a flush is no different from an injection once an extension is in place, and how hand injection protects against hydraulic dissection in complex coronary interventions.

🫀 CardioNugget™ | Mediastinal Radiation
Thickening + calcification of the aorto-mitral curtain (AMC) is a characteristic echo finding of prior mediastinal radiation.
The AMC = fibrous continuity between the aortic and mitral valves → radiation causes progressive fibrosis → thickening → calcification.
💡 Why it matters: Extensive AMC calcification reflects more advanced radiation-associated cardiac injury and has been associated with increased mortality after cardiac surgery.

I created LearnCoronary (https://t.co/VtlXhrUHJv), an interactive tool designed to help general cardiology fellows and other early-career learners better understand coronary angiography and the spatial relationships between different angiographic views.

The goal is flow, not a perfect angiography. 🩸
Dr. Riyaz Bashir (@riyazbashir) takes Dr. Hady Lichaa (@HadyLichaaMD) through balloon pulmonary angioplasty for CTEPH: guide shapes for each lobe, pressure wire guided balloon sizing, and why he deliberately undersizes in the first sessions.
Plus, what to do when a patient coughs during the case.

How long do you fight a backing-off guide in complex PCI before escalating to an extension catheter?
Dr. Sripal Bangalore (@SripalBangalore) joins Dr. Hady Lichaa (@HadyLichaaMD) to explain why that decision should take seconds, not minutes. They share tips for guide selection, positioning, and the safest ways to advance extensions through tricky anatomy.

It was really fun to make the free movement - the video doesn't do it justice, but you can free move both the TTE and TEE probe and it'll generate the 2D image in according to the 3D model.
There's a couple of chapters on common echo concepts and common diseases. Would love feedback!
