تم اعتماد اسم علمي جديد وأكثر دقة (وإن كان طويل وصعب الفهم لغير المختصين): "متلازمة المبيض الأيضية متعددة الغدد الصماء"
❌ الاسم القديم يوحي بوجود أكياس مرضية في المبيض، وهذا غير دقيق ويساهم في تأخير التشخيص.

مشكلتنا الأزلية طلب الأشعة بدون داعي طبي صحيح، تشخيص كسل الغدة لوحده لا يبرر طلب الأشعة. النتيجة؟ موال ماله أول ولا آخر من التحاليل والفحوصات والخزعات التي تضر أكثر مما تنفع.

🔸سبب تكون العقد داخل الغدة الدرقية غير معروف و يكتشف الغالبية العظمى منها عند اجراء اشعة صوتية للرقبة لاسباب عدة منها:
🔸وجود كتلة او تضخم بالغدة بالفحص الاكلينيكي
🔸وجود الم بالرقبة
🔸 صعوبة في البلع أو إحساس بوجود “شيء عالق”
🔸 بحة أو تغير في الصوت
🔸 ضيق في التنفس (خصوصًا عند كبر حجم العقدة)
🔸نسبة وجود سرطان بهذه العقد ضئيل جدا و لم يتجاوز ١٪؜ في دراسة نشرت عام ٢٠٢٢ و اجريت على ١٧٠٠٠ شخص لديهم عقد حجمها فوق ١ سنتيمتر تم اجراء خزعة لنصفهم ،،
🔸العقد تحت ١ سم ليس لها اهمية ،، لا تشغل بالك بها
🔸خصائص هذه العقد على الاشعة الصوتية يحدد الحاجة لاجراء خزعة من عدمها ،، الطبيب المختص هو من يقرر ذلك ،،
تسبب العقد قلقا للكثيرين مع ان الغالبية العظمى منها حميد و ليس لها تأثير على الصحة

APPROACH TO ANEMIA

Most anemia algorithms start with the MCV.

That’s a mistake.

The first branch point is the reticulocyte count.

It asks a physiological question:
Is the marrow responding?

MCV describes appearance.
Reticulocytes reveal function.

Together, they’re a solid team. But Opus has been a bit sloppy lately.

It’s also refusing more requests
