46yo male patient with HLA-B27+ SPA, compete ankylosis of spine and complete ankylosis of SIJ, still had IBP and CRP 20-23mg/L. STIR T2 MRI spine-no BMO. Spectacular improvement with TNFi!
The patient has indeed what looks like a Jaccouds arrhropathy and if you look carefully chondrolysis of the wrists.BUT! She has high positive anti-CCP and ANF 1/320 with no specificity. Clinically, no other signs of SLE. Overlap?
The itchy adherant scaly rash was quickly progressive and cultures for Staph aureus and Staph agalactiae were positive. There was spectacular improvement with Staphicid. It was an infection!
