Off out to the community this morning to follow up a lady we enabled to go home with single handed care- saving her 7 days in hospital and getting her home to her daughter quicker. All over a great result 👍🏻

@kadieclaire_OT Your community experience will be invaluable to use in acute. It’s much easier in an acute setting when you are able to picture your patient in their home environment and as a result what you might need to support them with. Acute placement is nice to have but not everything 👍
