The reopening of old, previously healed wounds (a phenomenon known as wound dehiscence) is caused by severe Vitamin C deficiency (Scurvy).
Pathophysiological Mechanism
Cofactor for Collagen Hydroxylation: Vitamin C (ascorbic acid) acts as an essential cofactor for the enzymes prolyl hydroxylase and lysyl hydroxylase. These enzymes are responsible for the post-translational hydroxylation of proline and lysine residues in procollagen.
Triple-Helix Stabilization: Hydroxylation is critical for forming inter-chain hydrogen bonds that stabilize the collagen triple-helix structure. Without functional collagen synthesis, cross-linking fails, leading to unstable mature collagen.
Continuous Turnover of Scar Tissue: Scar tissue is not static; mature collagen undergoes constant ongoing degradation and synthesis (remodeling). In severe deficiency, new collagen cannot form to replace degraded tissue, leading to the breakdown of existing scars and the reopening of old wounds.
