**Request for Regulatory Investigation, System Audit, Financial Reconciliation / Accounting Lapse Audit and Governance Review of Aditya Birla Health Insurance**

Respected Sir/Madam,
I respectfully submit this complaint seeking regulatory intervention into serious policy administration irregularities, operational lapses, financial reconciliation concerns, data integrity issues and possible control weaknesses observed in my dealings with Aditya Birla Health Insurance.

This complaint is not limited to an individual grievance. It raises broader concerns regarding policy issuance controls, transaction integrity, customer record management, internal audit effectiveness and governance practices which may potentially affect other policyholders as well.

Case Summary
A valid health insurance policy bearing number 21-22-3813390-03 was issued on 08 January 2026 as a Family Floater policy with coverage of Rs 45 Lakh comprising Rs 30 Lakh Base Sum Insured and Rs 15 Lakh Bonus Sum Insured.

Subsequently, another policy bearing number 21-22-3813390-04 was generated by the insurer reflecting materially different details including coverage of Rs 20 Lakh Individual comprising Rs 10 Lakh Base Sum Insured and Rs 10 Lakh Bonus Sum Insured along with premium discrepancies.

This policy was subsequently deleted, cancelled, rebooked or marked inactive by the insurer.

On 08 May 2026, an executive of the insurer informed me that premium had been received during a lapsed period and that a Fit and Fine Declaration was required for policy issuance.

Thereafter another policy bearing number 21-22-3813390-05 was issued on 11 May 2026.

The insurer subsequently admitted through written communication that a system related issue and operational lapse had occurred.

Till date they are not able to provide actual transaction details of my payment with time stamp. Similar way they are repeatedly saying my access amount has refunded and rejected by source. But not able to share complete details of my refund indent. Looks lot of accounting issue related to financial transaction data.

Major Regulatory Concerns

The insurer has admitted that an operational lapse occurred. However, despite repeated requests, the insurer has not provided:
a. Detailed root cause analysis
b. Internal audit findings
c. Accountability of responsible departments or officials
d. System logs and audit trail of policy creation, modification, cancellation and reissuance
e. Complete transaction reconciliation records
f. Actual payment timestamps and accounting entries
g. Detailed refund calculation methodology
h. Clarification regarding claim liability had a hospitalization occurred during the period when customer facing records reflected incorrect coverage

Potential Governance and Audit Concerns

The sequence of events raises several questions requiring independent review:

How could a Family Floater policy of Rs 45 Lakh be reflected as an Individual policy of Rs 20 Lakh for an extended period?

What controls failed within policy issuance and policy administration systems?

Were internal validation controls, maker checker processes and audit controls functioning effectively?

How was an incorrect policy allowed to remain active for months?

Why was a customer informed about lapse related requirements if the insurer now claims uninterrupted coverage?

Why do policy records, communications and subsequent corrections appear contradictory?

Whether accounting, premium booking and refund reconciliation controls were functioning correctly.

Whether similar operational failures may have affected other policyholders.

Public Interest Concern

Health insurance policies are relied upon during medical emergencies. Policyholders depend upon official policy documents issued by insurers. If customer facing records materially differ from internal records for extended periods, this creates significant consumer protection and governance concerns.

Relief Requested

I respectfully request:

Independent regulatory investigation into this matter.

Comprehensive audit of policy administration systems and controls.

Financial reconciliation and transaction audit relating to premium collection, policy booking and refund processing.

Review of internal governance, accountability and control mechanisms.

Examination of whether similar issues may have impacted other policyholders.

Appropriate directions and corrective action against any identified deficiencies.

Suitable compensation for the hardship, inconvenience, repeated follow ups and mental distress caused due to admitted operational lapses and prolonged uncertainty.

I am enclosing supporting policy documents, correspondence, payment records and grievance communications for examination.

I request the matter be treated as a serious governance, audit and consumer protection concern requiring detailed investigation.

WARNING FOR HEALTH INSURANCE CUSTOMERS (Beware from Aditya Birla Health Insurance)

Despite paying my renewal premium for @abhealthin on time, my valid active Aditya Birla Health Insurance policy (#21-22-3813390-03) suddenly disappeared from their system after renewal payment on 7th Jan 2026 while my previous policy was valid till 29th Jan 2026.
A wrong policy (#21-22-3813390-04) was allegedly generated internally, records were altered, complaints were repeatedly closed without correction, and customer care even claimed my renewal was pending despite policy activation already being done. Later they removed this also after my multiple follow-ups.
Big blunder, customer care later claimed renewal premium is paid on 5th May 2026 and lower premium amount shown rather what I actually paid on time.
I was left wondering — what if I needed hospitalization during this period?
Health insurance companies cannot play with customer medical security.
If this can happen to one customer, how many others may be affected?
Requesting @IRDAI_Official to investigate this matter urgently.
Also requesting @IRDAI_Official to conduct a serious audit of Aditya Birla Health Insurance systems and customer policy handling. Health insurance is about trust and security during emergencies — not stress and uncertainty.
Requesting media also to investigate this serious health insurance policy handling issue affecting customer trust and security.
