So how do I think about management for pts favoring surgery?
First- we must prioritize patient’s interests and goals. Balance of cancer control and quality of life is key! RT omission in appropriate patients is clearly a good option- algorithm here!
NICHE-2 absolutely delivers. Game over for chemo in dMMR colon cancer 💥🧬
In locally advanced dMMR colon cancer, FOxTROT already showed us chemo doesn’t work, and now NICHE-2 makes it undeniable.
💊 Neoadjuvant nivolumab + ipilimumab →
• 99% pathologic response
• 67% pathologic CR
• Zero recurrences to date at 3 years
🪦 This is the final coffin nail for perioperative chemotherapy in this setting. ATOMIC is already on life support despite being published less than a year ago.
The real question now: with Cercek’s rectal data, can we safely omit surgery next?
At minimum, this should be the new standard.
At maximum, it’s the start of a paradigm shift.

⚖️ TNT in Rectal Cancer: Promise or Premature?
💊 Total Neoadjuvant Therapy (TNT) has been hyped for LARC, but does it really beat standard chemoradiation (CRT)?
👥 4 Key RCTs:
•RAPIDO → ↓ distant mets, ❌ no OS gain
•PRODIGE-23 → ↑ DFS, small OS edge (but early curve drop & underpowered)
•POLISH II → initial OS benefit → vanished at 8 yrs
•STELLAR → ↑ 3-yr OS (86.5% vs 75.1%), but short f/u
📊 pCR rates: higher with TNT (22.5% vs 13.3%) but confounded by shorter CRT–surgery interval.
🚨 Takeaway: TNT = modest ↓ distant mets, unclear OS benefit, more toxicity. CRT remains standard until better biomarkers guide who truly benefits.
