Don’t just tell half the story, Mr Streeting.
You have also allowed a situation where qualified doctors are effectively being replaced by cheaper ACPs who cannot provide the same level of care.
You have offended residents by making sure their pay as fully trained doctors is lower than that of people “playing doctor” some of whom are unregistered and unaccountable.
You have created a doctor bottleneck with fierce competition for training posts, leaving many doctors unemployed - while at the same time some nurses are being pulled from already short-staffed nursing rotas to cover doctor roles, which only worsens the staffing crisis on both sides.
If you want to explain the reasons behind these decisions, the public deserves to hear them too. If the goal is to reduce the number of doctors in the NHS and shift to a workforce mainly made up of ACPs, then be honest about it. That would inevitably create a two-tier healthcare system. But the knowledge will at least give the opportunity for the public to start saving for private healthcare fees or accepting they will not get good care if they are poor. And young British people can make informed decisions about whether to study medicine in the UK and, if they choose to, to be clear they need to plan to emigrate early - unless they have the rainbow unicorn level of luck currently needed to secure a specialist training post.
I am not sure how many future doctors will want to become consultants in ten years’ time if we end up with no residents, no staff grades, no associate specialists, and a system full of unregulated non-doctors taking on doctor roles while consultants bear the legal responsibility. I certainly won’t be part of that - and good luck to those who choose to be. The lawyers won’t go easy on you - I promise you that.

On Friday, together with @Dr_WilkinCrowe, @NUHDREEAM and a brilliant faculty we ran the first M-CREST (Maternal Cardiac Resuscitation and Emergency Skills Training) course — improving ICU team preparedness for maternal cardiac arrest.
Faculty came from obstetrics, emergency medicine, intensive care, prehospital medicine, neonatology and human factors providing training in resuscitative hysterotomy, neonatal life support, and non-technical skills.

Doctors are taxpayers and NHS staff. We can help reduce waiting lists.
But successive governments’ failure to address our pay and training crisis mean that we have no choice but to strike. Unless you make a credible offer.
