@JamesTitcombe I do not think we talk about these concepts enough. How do people want to work? How do they actually have to work in these dysfunctional systems ? What are the barriers and obstacles. We do not have these conversations enough in the workplace. Therefore keep on blogging.
@JamesTitcombe The problem with reviews is they create inertia everyone waits around for the results fearful of what will be said concerned about whether they will be able to implement the change. I therefore like your idea that a potential national review would work with and alongside people.

@JamesTitcombe The investigations and reviews do not adopt restorative approaches and βimprovementsβ are not co-designed in partnership with families and the maternity staff doing the work. Therefore it is hit and miss as to whether improvements are embedded and sustained.
