Hello @Safeway I was trying to find a way to submit an issue online and couldn’t. I opened this Open Nature yogurt today, 5/3/23, and noticed an odd taste when I licked the yogurt off the lid. Then saw it had a thin fuzzy white growth. Expires 5/27.

@kara0ke_k8 @SFdirewolf done at a different office, they won’t send it with the records. They’ll tell you to request it from where it was done. So to get complete records, you have to request them from every office where the person was seen/treated. 2/2

One thing to know, the office will only send notes from their doctor. So even if they have a copy of an imaging report, if it was 1/
