Doxycycline. 100mgs. BID. 6-12 weeks. Under Dr. supervision. But tetracyclines are well tolerated.

Can be used in conjunction with something like accutane, but only for the first 4 weeks of dual therapy. Avoid long term overlap as this increases risk of intracranial hypertension.

I have terrible “bacne” from high dose DHT or EQ (the worst!) and this is the only therapy that clears my skin.
Takes time.

I also use DMSO directly on the acne. Helps a lot. But this is cystic acne and it requires a systemic intervention.
