from my archives. This little pt had HOCM; was on beta blockers; not compliant; came with chest pain. Initial echo showed his usual severe HOCM. Within the next 48hrs hours, his echo changed to this. no fever. high crp. unrelenting chest pain. Tapped him: thick purulent fluid
