Defining T-cell lymphoma doesn’t have to be complicated.

Start simple:

🔴 CD30+ → ALCL
 ALK+ vs ALK− (DUSP22 / TP63 / triple negative)

🔵 If not → check TFH markers
 PD1 / ICOS / CXCL13 / BCL6
 → often behaves like autoimmune disease

⚪ Only then → PTCL-NOS

🧬 Biology matters:
ALCL ≠ one disease
TFH = TET2 / DNMT3A / RHOA

👉 “CD30 first. TFH second. NOS last.”

📄 Splenic Irradiation for Myelofibrosis before HCT – Global Study (59 pts)

🔹 Rationale: Massive 🫛splenomegaly in MF → graft failure, poor graft function, relapse risk.
🔹 Intervention: Splenic irradiation (median 7 Gy, 5 fractions) within ~2 weeks pre-HCT.
🔹 Efficacy:
•97% response ✅
•Median spleen ↓ 5 cm
•Not dose-dependent
🔹 Toxicity:
•Main AE: thrombocytopenia & leukopenia 📉
•No major hemorrhage 🚫
•5% VOD post-HCT
🔹 Transplant outcomes:
•Engraftment: Neutrophils 92%, Platelets 81%
•3y OS: 62%
•1y NRM: 26%
•3y Relapse: 13%
🔹 Risk factors for worse OS: Severe anemia, severe thrombocytopenia, ↑MTSS, portal vein thrombosis, high-intensity conditioning ❗
🔹 Comparison:
•Relapse at 3y: 12% (irradiation) vs 29% (immediate HCT) vs 38% (splenectomy) 📊
•OS similar across groups (~61–63%)

✅ Conclusion: Splenic irradiation is feasible, reduces spleen size rapidly, lowers relapse risk, and is a reasonable option for MF patients with JAKi failure prior to HCT.
