I have looked at brains under the microscope for many years now, but knowing that neurons and glial cells can fuse with each other is something I never learned during my Ph.D. We learn that most viruses don't even directly cause damage to the brain, and when they do, it is only because they killed their host cells. However, some chronic neurological sequelae of viral infections cannot be explained only by neural cell death.
It is known that in non-neuronal tissues, viruses fuse with host membranes to enter cells, hijacking the cellular machinery to produce viral components and sometimes conferring the ability to fuse with neighboring cells. This results in the formation of multinucleated cells, allowing viral propagation without needing virion release into the extracellular space.
Guess what? Viral-mediated fusion of brain cells also exists but still remains poorly understood.
The irreversible fusion of brain cells causes disruption in neuronal communication and reveals another possible mechanism to explain chronic neuronal malfunction even many years after the initial infection.
We now know that #SARSCoV2 can also cause brain cells to fuse, especially inducing neuron-neuron fusion. Instead of firing independently to propagate signals throughout the brain, fused neurons fire simultaneously, and this massive amount of synchronous activity is similar to seizure-like events. The desynchronized neuronal activity caused by fused neurons can also help explain symptoms like brain fog. When these events occur in the autonomic nervous system, for example, it can help explain dysautonomia, which so often happens with #LongCovid patients.
We must keep fighting against the concept that the brain is an isolated, immuno-privileged site.
Immune privilege, a term coined by the Nobel Prize winner Peter Medawar in the 1940s, holds that a few sites in the body, including the brain, do not show typical inflammatory responses, an evolutionary adaptation to protect these critical sites from potentially damaging inflammation. This could not be more far from the truth. Unfortunately, evolution did not give us this unique ability. This dogma has only served to bias the interpretation of our findings and slow the discovery of basic mechanisms induced by the presence of pathogens and potential therapies to counteract them.
