Nektar Rezpeg binds to CD25 or the IL-2R alpha subunit, this activated tTreg cells (natural or thymal Tregs) and makes them proliferate and control inflammation as seen in AD and AA. However, regular dosing at Q4W seems necessary to keep this treatment effective as still a Signal 3 pathway so the tTregs will need constant stimulation (period to still be determined).

Conversely I am postulating that Corvus Soquelitinib by inhibiting ITK is making iTreg cells (induced or peripheral Tregs), this was shown by Avery August and Corvus, and by affecting the Signal 1 pathway is actually making long-lasting iTregs that are stable and do not seem to need regular stimulation or boosts to see benefit. This could be explanation why 90% patients still showed AD remission 90-days after treatment stopped. This suggests epigenetic changes that are making a long-lasting iTreg and a more complete rebalancing of the immune system.
