@TCOrobin I was just wondering how she was doing. Condolences to her family, all who knew her and all who were moved by her. She made a difference in patient safety and more broadly in healthcare.
The implementation of Martha’s Rule will take a phased approach, beginning with at least 100 adult and paediatric acute provider sites who already offer a 24/7 critical care outreach capability. We will soon be asking for expressions of interest to be part of the first phase.
This is a great opportunity to commit to a patient safety culture -a just and learning culture-that is supported by leadership and measured by performance accountability. See the book: Fatal Solution
Thanks to everyone who took part in the first Martha's Rule working group today, one of the key elements of my new strategy https://t.co/Nh827GBB4p - looking forward to progressing this initiative with this dynamic group

Thank you for doing this work. My mother would have been helped if there was a rapid response team that could be initiated by family. Failure to rescue cases can be prevented. #ptsafety
It turns out, they are not pursuing mediation for legal reasons but rather emotional ones. Mediation is an opportunity to help the family accept their decision, and to achieve peace of mind and closure before leaving this world. #healthlaw#mediation#MAID
In Kamloops and have a @Budget car rental. Office staff at the airport and Notre Dame locations are what good customer service should be like!! I’m a fan. Thank you!!
Organizations need to have wholesome conflict management strategies to prevent an incident escalating to institutional betrayal, moral injury, to moral rage that comes to social media. #justculture
I have represented a family pro bono at a Fatality Inquiry and there was so much more to be brought out than what the Crown could see related to patient safety that they knew nothing about. #ptsafety

It’s about ensuring the system and individuals are considered and all possible recommendations with respect to safety are set out. And the survivors are heard for their perspective. #ptsafety#justculture
I’m so honoured to have been part of this project! We need transparency in patient safety. We need legal reforms for those harmed. Thanks to all the patients/families who have been part of positive change.

I have no words. Thanks to the
@thetimes who led with the story about 1/3 female surgeons being sexually harassed, even raped, at work, for
publishing this letter from an anaesthetist saying we should expect it!!
This is why it is still goes on
@wpsms_org@RCoANews
Bullying should not happen. Sexual harassment is a criminal offence NOT a routine part of medical training
