@nihardesai7 Not sure this is EMD as others have suggested. Typically chest wall disease is bone based. Regardless, I think the best option would be alkylator-based therapy. I would probably give VCd for 2-4 cycles (if refractory, then pivot to DCEP) followed by ASCT.
@bdermanmd@rajshekharucms 10 days late to the discussion but I was going to make the same comment, @bdermanmd. With a growing number of ways to define high risk, most will have at least one high risk marker eventually. And when everyone is high risk, no one is.
The diagnosis of osteoporosis often leads to a preventable visit to a cancer center, and a long period of existential distress about having cancer.
Please dont check MGUS/MM labs as part of secondary osteoporosis workup.
MGUS does NOT lead to a decrease bone density!

@DrKLindley @AaronGoodman33 Coincidentally I’ve seen two recent intravascular lymphoma patients whose diagnosis was based in part on FDG uptake in the uterus, one with large cells on Pap smear!
I recognize that many people who have done more than I ever ever have worked hard on this. I also recognize that compromises have to be made and perhaps industry dictates design.
But if CLL can have perfect co-op trial to answer this, it breaks my💓to see that we cant.
