systematic review and meta-analysis evaluated the diagnostic performance of PSMA PET imaging in detecting local or metastatic disease in 8,119 #ProstateCancer patients experiencing biochemical recurrence (BCR) following radical prostatectomy or radiation therapy. The analysis revealed that PSMA PET positivity rates varied significantly based on prior treatment type, PSA levels, and lesion location, with higher detection rates seen after radiation therapy (92%) compared to prostatectomy (60%). Positivity increased with rising PSA levels, reaching over 90% at PSA >2 ng/ml🧪. Detection was most common in pelvic nodes and local recurrence, while visceral metastases were rare.
Study heterogeneity limits generalizability, but the findings underscore the importance of PSA stratification and highlight PSMA PET’s potential to inform salvage treatment decisions, especially in early recurrence.
