Endnotes from ID are a total pain, as you well realize. I recommend converting them to footnotes before conversion to EPUB. Post-export you can move them around, if you like.
We tend to use italics for one of two reasons: for emphasis, or to convey an alternate voice (such as direct thought in narrative fiction). Here’s a nice, short exploration of styling italics and bold semantically instead of presentationally:
