Never assume they are pain seeking

- normal retic count and Hb do not rule out “real” pain

- do a thorough extremity and joint exam. They are high risk for septic arthritis and osteo

- Bicuspid AV is risk factor for dissection

- phenyleph for critical AS, levophed and/or dobutamine for AR/MR

- involve CT surgery early with sick pts

- prosthetic valve dysfunction is thrombosis until proven otherwise

Don’t forget valvular causes! If comfortable with echo and there is critical AS —> phenyl is pressor of choice
- severe MR —> CT surgery or interventional
- severe AR —> try and visualize aortic root for dilation
