This is a trap!
We should always start from 2D and 2D color views and then use 3D just to ameliorate our evaluation.
In this case 3D cannot allow a diagnosis!

This is a endocarditis vegetation on P1-P2M1. 2D color shows only mild mitral regurgitation.
