#Procalcitonin (PCT) may be a potential inflammatory marker in the course of Still’s disease (SD), as demonstrated by a retrospective study that included 110 patients with SD and absence of infection:
◦ PCT levels were significantly higher than in healthy controls and nearly as high in macrophage activation syndrome (MAS) cases as in confirmed infections.
◦ 32% had baseline PCT ≥0.5 ng/ml; 43% had peak PCT ≥0.5 ng/ml.
◦ Elevated PCT (≥0.5) was strongly linked to:
- Higher frequency of MAS
- Higher mPouchot scores
- Greater use of biologics (especially anakinra)
- Laboratory signs of severe inflammation (thrombocytopenia, ↑ AST/LDH/CRP/ferritin, ↓ albumin)
◦ MAS risk was independently increased:
- ~4.3-fold (baseline PCT ≥0.5)
- ~5.4-fold (peak PCT ≥0.5), after multivariate adjustment
◦ PCT normalized with immunosuppressive treatment.
Although PCT cannot be used to distinguish active SD from infection, these findings suggests that increased PCT levels in patients with SD are associated with a severe inflammatory response. PCT may also serve as a potential independent predictor of MAS and could be considered a convincing marker in the assessment of non-infectious inflammation.

Anti-Ro/SS-A and anti-La/SS-B #autoantibodies occur in ~2/3 of #Sjögren’s disease (SjD) cases and are key for diagnosis. However, ~1/3 of patients are seronegative for these, often requiring minor salivary gland biopsy for confirmation.
Emerging evidence supports novel autoantibodies for diagnosing anti-Ro/SS-A-negative SjD. Many (e.g., anti-ASCA, anti-TRIM38, anti-PUF60) associate strongly with anti-Ro/SS-A positivity, limiting seronegative utility. Promising candidates include:
◦ Anti-muscarinic M3 receptor (anti-M3R), showing high sensitivity/specificity.
◦ A 12-autoantigen panel (e.g., GMNN, GRAMD1A, NUP50) with 54% sensitivity and 100% specificity in anti-Ro/SSA-negative SjD.
◦ Validated panels: 3-marker (anti-FNBP4, anti-SNRPC, anti-CCL4) detects ~30% of anti-Ro/SSA-negative cases (specificity up to 97% vs. controls/non-SjD sicca); 5-marker (adding anti-M3R, anti-KDM6B) reaches 46% sensitivity at 95% specificity.
Other markers like anti-NA14, anti-calponin-3, anti-CCL4, anti-M5, anti-TMPO, anti-OAS3 show potential, with some (e.g., anti-TONSL, anti-IL6) linked to extraglandular features like pulmonary involvement.
Recent 2024–2025 studies confirm these panels' value in identifying seronegative SjD and predicting abnormal gland pathology, potentially reducing invasive biopsies. Larger validations and integration into diagnostics continue, advancing non-invasive tools for earlier, accurate classification in seronegative cases.
