I learnt this from my trips to the Indian subcontinent. A small plastic bag attached to the biopsy port of duodenoscope during ERCP to collect the spurting bile. These simple clever innovations make so much of a difference. (I need to attach a smaller bag next time) #GITwitter

ESD of a Descending Colon Polyp! 🌟

📊 Type: Paris classification 0-IIa+Is.

🔬 Observations:

Healthy submucosa found beneath normal mucosal margins (black arrow)
Fibrotic Submucosa (orange arrow) under the lesion-unusual presentation in a polyp not previously attempted.
Pathology indicated invasive adenocarcinoma to the superficial submucosa.
En-bloc resection, Clear lateral and vertical margins, with no lymphovascular invasion.
🤔 Despite the patient's youth and clean margins, the presence of fibrosis, often indicating advanced pathology, prompted a referral to colorectal surgery.

✔️ Outcome: A left hemicolectomy was performed, with 1 out of 26 lymph nodes testing positive for CA.
