How to report intestinal metaplasia when you receive specimens labeled as
-Distal esophagus: Barrett's esophagus (BE)
-Squamocolumnar junction or Z-line: Consistent with Barrett's esophagus if biopsies were taken proximal to Gastroesophageal junction (GEJ)
-GEJ: Do not diagnose Barrett's unless you see esophageal ducts or glands (see next post 👇). Say in a comment the differential diagnosis includes BE and intestinal metaplasia of gastric cardia

Esophagus-p53 IHC pitfalls-Barrett's Esophagus
1. p53 should not be used to grade dysplasia
2. All high grade dysplasia does not show a mutated pattern
3. Not useful at the two ends of the spectrum (if you are sure of the diagnosis by H-E)

One of my favorite diagnoses in #GynPath: corded and hyalinized endometrioid carcinoma (CHEC)
⚠️Not to be confused with carcinosarcoma
Corded/hyalinized/spindled component of CHEC shows:
▶️Low-grade cytology similar to glandular component
▶️Wild-type p53
▶️Nuclear beta-catenin
